If you are a new patient to our office, please visit our Patient Forms page to download our new patient forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your dental needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

We kindly ask that payment be made at the time of your visit when services are performed. For your convenience, we accept cash, personal checks, and most major credit cards. We also offer dental financing from Care Credit.

For a flat annual fee, you’ll receive:– Two comprehensive exams – Two professional cleanings – Yearly X-rays – Fluoride treatments – Plus 15% off all additional dental treatment.  It’s a simple, affordable way to keep your smile healthy all year long—no insurance needed!
